I, the undersigned, acknowledge that I am voluntarily participating in the Amazing Grace Baptist Church Round-Up Carnival Sunday and understand that participation may involve certain risks. By signing this form, I agree to the following: I assume full responsibility for myself and any minor child(ren) under my care while attending this event. I understand that Amazing Grace Baptist Church, its pastors, staff, volunteers, and representatives are not responsible or liable for any accident, injury, illness, medical emergency, loss, theft, or damage to persons or property that may occur before, during, or after the event. I acknowledge that I am responsible for the supervision and safety of my child(ren) and any guests attending with me. In the event of a medical emergency, I authorize emergency medical personnel to provide treatment if I cannot be reached. I understand that I am responsible for any medical expenses incurred. By signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms of this Liability Waiver and Consent Form.
